All Care Names New Adult Foster Care Program Manager

Michele Meltzer, LICSW, of Swampscott has recently been named Adult Foster Care Program Manager for All Care’s Resource Division.  Meltzer, who has Master’s Degree in Social Work from North Carolina Agricultural and Technical State University, joined All Care in November 2016 as a VNA Medical Social Worker before being promoted to Adult Foster Care Manager in April of 2018. With over 15 years’ experience in social work, Michele brings a wealth of experience to her new role at All Care. All Care’s Adult Foster Care Program (AFC) allows individuals, who are unable to remain at home alone due to chronic illness, or physical and/or mental disabilities, to stay in their home or in the home of a loved one to maximize their quality of life. The Adult Foster Care program is an alternative to nursing home or assisted living placement. In-home family caregivers provide meals, companionship, personal care assistance, and 24-hour supervision to program participants. Caregivers receive a tax-free stipend which can help to ease the financial burden of caring for the individual. The AFC program provides monthly nursing and social work visits where both client and caregiver receive the support and resources they need to share a safe, happy, fulfilling life. Community Involvement “Makes the Mark” for All Care Hospice at the 26th Annual All Care Charity Golf Open

LYNN – All Care VNA & Hospice announced that its 26th Annual Charity Golf Open held on June 25th at Gannon Golf Course, Lynn, MA was a tremendous success, thanks to the involvement of many members of the community. Over $50,000 was raised from the event, which will benefit All Care Hospice patients by providing compassionate medical, emotional and spiritual care, regardless of one’s ability to pay. Personalized bereavement support is provided to families, in addition to support groups available to the community at the All Care Hospice Center for Grief & Loss, located at the agency’s main headquarters at 210 Market Street in Lynn.   “A little rain definitely didn’t dampen the spirits of our golfers who ‘made their mark’ for All Care Hospice,” notes Julie Manninen, All Care’s Manager of Development and Fundraising. “We were excited that this year’s event was very well attended, with many new golfers joining us for the first time!  We were also grateful for the loyal support of many of our returning sponsors as well. All Care Names New Palliative Care Program Manager

Laurie McFarland, RN of Winthrop has recently been named Palliative Care Program Manager for All Care’s Hospice Division. McFarland joined All Care in 2016 as a Hospice RN Case Manager before being promoted to Palliative Care Program Manager in April of 2018. She served as a hospice and palliative care nurse at Hallmark Health in Malden for five years, and as staff nurse focusing on end of life care at Epoch of Melrose for two years. McFarland brings a great deal of experience to her new role at All Care and will oversee the Palliative Care Program ensuring our patient’s needs are met, their symptoms are managed and the highest quality care is delivered. All Care’s Palliative Care Program provides home care services for patients with advanced illness. The Palliative Care Program introduces an interdisciplinary team of professionals, which emphasizes palliative services, while addressing the many ways serious illness affects patients and their families. The team includes a nurse practitioner, registered nurses, social workers and a psychologist. The Palliative Care Program Nurse conducts periodic visits providing care and consultation in pain and symptom management. All Care Hosts Annual Meeting, Honors Employees

All Care VNA, Hospice & Private Pay Services, headquartered in Lynn, MA, has been providing the highest quality home health care to individuals in the North Shore, Merrimack Valley, and Greater Boston communities for over 100 years. On Wednesday, May 2, 2018, All Care board members and staff gathered together for the agency’s Annual Meeting at Spinelli’s in Lynnfield.  In addition to celebrating All Care’s many achievements over the past year, Excellence Awards were presented to three employees for their exceptional job performance and their commitment to All Care’s mission.  The Josephine E. Hanson Award for Excellence in Professional Achievement was presented to Payroll Coordinator, Catherine Drinkwater of Salem, MA; the Dorothy L. Lane Award for Excellence in Outstanding Patient Care was presented to Hospice Nurse, Caroline Ramos of Nashua, NH; and the President’s Award for Excellence in Para-Professional Achievement was presented to Home Health Aide, Carline Desrosier Exama of Lynn.  Many employees also received milestone awards for their years of dedicated service with All Care: 22 employees received 5-year awards; 13 employees received 10-year awards; 6 employees received 15-year awards; 1 employee received a 20-year award; and 5 employees received 25-year awards. All Care’s Quality Services Recognized by the Department of Public Health

All Care Visiting Nurse Association (VNA) is pleased to announce that the Department of Public Health (DPH) has informed All Care that it has successfully passed its recent triennial DPH survey. The DPH surveyed All Care over a five-day period in January and because of the compassionate efforts of its dedicated staff, the agency passed without any deficiencies. All Care was also awarded high accolades, and was noted as being in full compliance, as it has been for many prior DPH surveys. All Care VNA has long prided itself on providing the highest-quality care and services to its patients for over 100 years. Each staff member works toward this goal every single day, according to Director of Quality Improvement, Mary Ellen Dawley, RN, BSN, COS-C. “Our home health aides, nurses, OTs, COTAs, PTs, PTAs, speech therapists and social workers work with our patients and their families to provide an individualized and specific plan of care. They also work in collaboration with the patient to assist them in working toward their goals. We work together at achieving those goals with a team approach, but the patient is always leading the charge,” says Dawley. The Department of Public Health conducts these surveys every three years with all certified home care organizations to ensure full compliance with the conditions of participation and guidance from the Center of Medicare and Medicaid services.
