All Care’s Adult Foster Care Program

All Care’s Adult Foster Care (AFC) program is a wonderful alternative to nursing home or assisted living placement. The program allows individuals, who are unable to remain at home alone due to chronic illness, or physical and/or mental disabilities, to stay in their home or in the home of a loved one in order to maximize their quality of life. In-home family caregivers provide meals, companionship, personal care assistance, and 24-hour supervision to program participants. Caregivers receive a tax-free stipend which can help to ease the financial burden of caring for the individual. The AFC program provides monthly nursing and social work visits where both client and caregiver receive the support and resources they need to share a safe, happy, fulfilling life. Sally is a perfect example of how the AFC program can help an elder remain and thrive at home.   Sally had been living independently in her own apartment but as her disease process progressed she experienced multiple hospitalizations. Her last hospitalization resulted in a short term stay in a rehabilitation hospital. During this time Sally experienced increased cognitive deficits and had difficulty caring for herself without physical assistance. It was recommended that Sally stay in the facility and permanently reside on the Long Term Care Unit. Fortunately, the social worker spoke to the family about the option of Mom returning home with the assistance of one of her children through All Care’s Adult Foster Care Program.
